No product selection indicated. Prescriber authorized generic substitution and a patient accepts the generic substitution or a generic equivalent is not available.

DAW 1

Substitution not allowed by prescriber. Prescriber requires that the prescription be filled exactly as written.

DAW 2

Substitution allowed. Prescriber authorized generic substitution and generic equivalent is available, but the patient requested that the brand product be dispensed.

DAW 3

Substitution allowed. Pharmacist selected for the brand product to be dispensed, even though a generic is available.

DAW 4

Substitution allowed. Brand product dispensed, because generic medication not in stock.

DAW 5

Substitution allowed. Pharmacy uses a branded generic.
