Critical Thinking
The mental process of active and skillful analysis, synthesis, and evaluation of information for decision-making.
Client-Centered Care
Care that reflects the highest possible level of wellness and independence for the client.
SMART Goals
Criteria for setting goals: Specific, Measurable, Attainable, Relevant, Time-Bound.
Subjective Data
Information reported by the client, such as symptoms described in their own words.
Objective Data
Observable data that can be measured or observed, such as vital signs.
Prioritizing Problems
The skill of determining which client issues need immediate attention in nursing.
High Priority
Problems that are life-threatening and require immediate action, often related to airway, circulation, or safety.
Intermediate Priority
Non-emergent problems that do not pose an immediate threat to life.
Low Priority
Issues that may not directly relate to the current medical condition but could affect future well-being.
Outcomes
The desired results or goals of nursing interventions that measure the success of care.
Individualized Care
Nursing care tailored to meet the specific needs and preferences of each client.
Rationales for Interventions
Scientific reasons supporting why specific nursing actions are chosen.
Implementation
The phase of the Nursing Process where planned interventions are carried out.
Inferences
Logical conclusions drawn from assessment data to identify client issues.
