Tissue Integrity
The ability of body tissues to maintain structure and function, often used in the context of skin health and wound healing.
Braden Scale
A tool used to assess a patient's risk for developing pressure ulcers, with a score of 23 total points where a score of <18 indicates risk.
Pressure Injury Stage 1
Intact skin with non-blanchable redness, indicating pressure injury at the earliest stage.
Stage 2 Pressure Injury
Partial-thickness skin loss with a shallow open ulcer, characterized by a red wound without slough.
Stage 3 Pressure Injury
Full-thickness tissue loss, where subcutaneous fat may be visible but bone, tendon, or muscle is not exposed.
Stage 4 Pressure Injury
Full-thickness tissue loss with exposed bone, tendon, or muscle, indicating severe injury.
Secondary Intention Wound Healing
Healing that occurs with significant tissue loss and open wound edges, requiring more time for healing.
Sanguineous Exudate
Bright red blood that indicates active bleeding in a wound.
Purosanguineous Exudate
A mixture of pus and blood found in newly infected wounds.
Complications of Wound Healing
Infection, hemorrhage, dehiscence, and evisceration that may complicate the healing process.
Malnutrition
A factor that can impair wound healing by lacking essential nutrients necessary for tissue repair.
