Prescription requirements
Name and address and telephone number of the person prescribing the product
Qualification enabling the person to prescribe (RCVS/SQP number)
Name and address of the owner/keeper
Identification (including species) of the animal/group of animal
Premises at which the animals are being kept if different from address of the owner
Date of the prescription
Signature/other authentication of person prescribing
Name and amount of product
Dosage an administration instructions
Any necessary warnings
Withdrawal period
If under the cascade requires an additional statement
CD prescription requirements (additional)
Prescription must be an original (typed or handwritten) and personal signed
Declaration that the CD is prescribed for animal/her under the VS’s care
Amount prescribed in words and figures
Dose to be administered (NOT take as directed)
