Prescription requirements

  • Name and address and telephone number of the person prescribing the product

  • Qualification enabling the person to prescribe (RCVS/SQP number)

  • Name and address of the owner/keeper

  • Identification (including species) of the animal/group of animal

  • Premises at which the animals are being kept if different from address of the owner

  • Date of the prescription

  • Signature/other authentication of person prescribing

  • Name and amount of product

  • Dosage an administration instructions

  • Any necessary warnings

  • Withdrawal period

  • If under the cascade requires an additional statement

CD prescription requirements (additional)

  • Prescription must be an original (typed or handwritten) and personal signed

  • Declaration that the CD is prescribed for animal/her under the VS’s care

  • Amount prescribed in words and figures

  • Dose to be administered (NOT take as directed)

  • RCVS number of VS

  • Repeat prescriptions for schedule 2/3 are not permitted-mark as no repeats

  • Must be dispensed within 28 days
