What are the two patient identifiers used to verify a patient before care?
Name and date of birth.
What is the recommended hand hygiene before touching a patient?
Wash hands or use sanitizer.
What should you do before performing any procedure on a patient?
Inform the patient about what you are going to do.
What does 'Oriented x3' mean in patient assessment?
The patient is oriented to person, place, and time.
What questions assess orientation to person, place, and time?
Questions like 'Do you know who you are?', 'Do you know where you are?', and 'Do you know what year it is?'
What signs indicate facial symmetry during assessment?
Ability to smile, absence of facial drooping, and ability to puff out cheeks.
What eye aspects are evaluated in a basic exam?
Overall appearance, symmetry, any drooping, redness, or drainage.
What does PERRLA stand for in eye assessment?
Pupils Equal, Round, Reactive to Light, and Accommodation.
What does EOM stand for and how is it tested?
Extra Ocular Movements; tested by following a pen light in up, down; left-up/right-down; right-up/left-down directions.
What should happen to the pen after each EOM movement?
The pen should return to the center after each movement.
