Hand Washing(need: water, soap, paper towels)

1. “Hi, my name is Jillian Sistrunk. I will be your CNA today. I will be preforming the skill of handwashing” (explain every step)

2. turn on water & wet hands

3. lather hands in soap and scrub for at least 20 seconds before rinsing thoroughly

4. rinse all surface, keeping hands below elbows

5. dry hands and wrists with paper towel and place in trash

6. turn off faucet with new paper towel and place in trash

knee high elastic stocking (need: TED hose)

5. turns stocking inside out up to the heel

6. place foot of stocking over toes —> foot —> heel

Assist to ambulate using transfer belt

12. walk slightly behind and to one side of pt for 10 feet while holding onto belt

Donning and Removing PPE

2. Pick up and unfold gown

3. put arms through each sleeve

5. secure gown at waist making sure that clothing is covered

8. with one gloved hand, grasp palm of other hand, remove glove

9. ball up removed glove in gloved hand

10. slips fingers from ungloved hand underneath cuff of remaining glove at wrist and remove glove to make it inside out

11. dipose of gloves

12. unfasten gown at waist and neck

13. remove gown without touching outside of gown by reaching into the cuff and balling it up from left to right arm

14. keep gown inside out as you roll it up

15. dispose of gown

dress client with affected right arm

3. present 2 shirts and ask which they would like

4. make sure pt’s chest is covered

5. remove gown from unaffected arm (left) side first then the right arm

7. assist to put affected arm (right) through right sleeve of shirt before placing unaffected arm (left)

feeds client who can’t feed self

3. ask pt to state full name

4. pt in upright position (75-90 degrees)

5. place tray in front of pt and sit in front of them

6. ask if they would like a clothing protector and place it on

7. clean pt hands before feeding (1 swipe for each side of hand)

8. ask what they’d like to eat first

9. offer pt one bite of each food on tray, telling them what it is

10. offer drink after every bite

11. ask pt if they are ready for next bite of food or sip of drink

12. at end of meal, clean pt’s mouth with clothing protector and use napkin with 1 swipe for each hand

13. remove food tray

14. leave pt in upright position

15. give call light and privacy if wanted

16. take tray and put on hallway cart

Give modified bed bath

2. put barrier of towel down so you can put basin on top

3. give privacy

4. remove gown and place directly in soiled linen, making sure pt is covered

5. put gown on chair as dirty barrier

6. check water temp is safe and ask client to verify the temp is comfortable

7. wash hands

8. put on gloves

9. wet towel and fold

10. wash eyes (without soap), using different corner of washcloth for each stroke (inner to outer aspect of eye), then using new corner go from bottom to top of face, drawing out E

11. dry eyes and then face using same motions and order

12. expose one arm and place towel underneath arm

13. applies soap to wet washcloth

14. wash fingers (fingernails too) and hand, then fold towel with each corner folded in and snake up arm up to shoulder

15. then repeat with wet towel

17. put on clean gown started with affected arm, placing gown over sheet and pull sheet out from under gown

19. empty & rinse water into commode, and dry basin and place in dirty section

20. live soap for pt

measure and record urinary output (±25ml/cc)

perform modified passive range of motion (PROM) for one knee and one ankle

5. grab under knee and at ankle and bend the knee and return to normal positon (repeat 3 times and ask if any pain every time)

6. support the foot and ankle and push/pull foot toward head and then base of bed(repeat 3 times and ask if any pain every time)

7. signal device in reach and make sure bed in low position

perform modified passive range of motion (PROM) from one shoulder

11. wet wash cloth and repeat

12. dry wash cloth and repeat

14. empty water into commode and rinse and then put in dirty area
