Injectable Drugs Card Admin and stability 1-20

Acetylcysteine

Must be diluted in dextrose 5% for IV infusion. Loading dose 200ml D5 and infused over 15 mins.

Acyclovir

Reconstitute by adding sterile water for injection to produce concentratoin of 50mg/ml. Further dilute to final conc of 7mg/ml

Adenosine

Give by rapid bolus IV onlly. Admin IV push over 1-2 seconds at a peripheral IV proximal to trunk

Alteplase, Recombinant

Reconstituted by adding sterile water for injunction to vial to conc 1mg/ml

Amikacin Sulfate

Undiluted for IM injection. IV infusion diluted 500mg with 100-200ml of solution of D5W, NS

Amiodarone

Dilute with 5% dextrose in water prior to use 1-6mg/ml. Use glass because absorbs in pvc.

Amphotericin B

50mg vial diluted with 10ml of sterile water to conc 5mg/ml. IV 0.1ml/mg peripheral and 0.25mg/ml for central catheter infusion

Ampicillin Sodium

Reconstitued with small volume of sterile water for im injection. do not freeze

Ampicillin Sodium and Sulbactam Sodium

Slow iv over 10-15 mins or greater dilutions with 50-100ml of caparible dilutent of 15-30mins. IM also. Reconstute with sterile water, lidocaine.

Atropine

IM autoinjection to outer thigh for 10 seconds. can go through clothing. IV rapid. Intrhecal dilute in water.

Azithromycin

Injections admin IV over a period of at least 60 minutes. Do not inject by IM or as IV bolus

Aztreonam

For directed IV injection, the dose should be diluted with 6-10 ml of dilutent, and injected over a period of 3-5 minutes.

Bumetanide

Parental admin by direct IV injection, IM, of IV infusion. Direct IV given slowly over a period of 1-2 mins.

Buprenorphine Hydrochloride Class III

Admin undiluted by deep IM injection and slowly by direct IV iinjection over a period of at least 2 mins. For continuous IV, durg should be diluted with NS to a conc. of 15mc/ml and admin by epidural injeciton at a conc. of 6-30mcg/ml

Butorphanol Tartrate Class III

Given by IM injection or direct IV injection undiluted.

Caspofungin

Infuse slowly over 1 hour. Store vials under refrig. Reconstitute vials using 0.9%. NaCL, sterile water, or bacteriosatic water for injection and furture dilute with .9%, 0.45%, or 0.225% Nacl or lactated ringers

Cefazolin Sodium

Reconstitued with sterile water, bacteriostatic, or NaCL 0.9+% for injection.

Cefepime

Maxipime in the dry state should be store between 2-25 C or 36-77F and protected from light.

Cefetaxime Soium

For IM injection, 2ml, 3ml, or 5ml of sterile water may be added to 500g, 1g, or 2 gram vials and admin deep muscle

Ceftraixone Sodium

Direct IV injections, vials should be reconstitured with sterile water to a concetration of 100mg/ml and injected over a period of 2-4 minutes.
