Step 1
- Verify Physicians Order
- Identify Current Glucose Value
- Select Insulin Syringe with small gauge needle (25-27)
Step 2
- Identifies correct prescribed insulin types: Regular and NPH.
- Verifies Expiration Date on Med Vial
Step 3
- Roll Cloudy vial in hands to mix solution (NPH)
Step 4
- Performs hand hygiene
- Swabs the top of both insulin bottles with alcohol wipe.
- Allow injection stoppers to dry
Step 5
- Injects Volume of air equal to prescribed dose (2mL) in FIRST vial NPH, and then (3mL) into second vial Regular without letting the needle touch the solutions.
Step 6
- Withdraws correct dose of Regular insulin; then with needle still inside vial, verifies amount with second nurse.
- Withdraws Needle from Vial
Step 7
- With the Same Syringe withdraws precsribed amount from NPH (cloudy) vial.
- With needle remaining in vial, verifies amount with second nurse.
- Withdraws needle from vial.
- Uses scoop method to recap needle.
Step 8
- Verbalizes maximum delay from mixing to administration (5 minutes: regular insulin binds with modified (NPH) insulin, thus reducing the action of regular insulin).
Step 9
- Preps skin with alcohol wipe, moving from the inner area to the outer in a circular motion.
- Allows area to dry
- Performs Subcutaneous Injection Technique.
