Every 15 minutes until reaching the ED.
How often should vital signs be reassessed in an unstable patient?
Every 5 minutes, or as often as the situation permits, monitoring for trends in the patient’s condition.

Yes, but the body’s ability to compensate may decrease over time (decompensated shock), and vital signs can deteriorate rapidly, especially in children.
Why is treating a patient for shock before obvious signs appear important?
It increases the patient’s chance of survival.
