Medication Administration Cross-Check
A verbal procedure used to verify the right drug, patient, and dose to reduce medication errors.
Step 1: Med check
The person giving the medication alerts their partner by saying, “Med check.”
Step 2: Partner ready
The partner responds, “Ready,” signaling attention to the verification process.
Step 3: Medication statement
The person giving the medication states, “I am going to give [medication name, dose, and route] for [indication].”
Step 4: Contraindications check
The partner responds, “Contraindications?” to confirm no allergies or reasons the medication should not be given; the first person replies, “None.”
Step 5: Volume verification
The partner asks, “Volume?” and reviews the medication container and dose measurement device; if correct, the partner says, “Give it.”
Solo verification
If no partner is available, the EMT can perform the cross-check procedure aloud alone in the ambulance.
