Nursing Diagnosis
A clinical judgment concerning human response to health conditions/life processes, or a vulnerability for that response, by an individual, family, group, or community.
Purpose of Nursing Diagnosis
Helps identify nursing priorities, direct interventions, formulate expected outcomes, and establish communication among healthcare professionals.
Difference between Nursing Diagnosis and Medical Diagnosis
Nursing diagnoses focus on patient responses to health conditions, while medical diagnoses address specific diseases or conditions that physicians treat.
Collaborative Problems
Potential problems that require both medical and nursing interventions, with nursing focused on monitoring and prevention.
NANDA-I Taxonomy
A system that includes 234 nursing diagnoses classified within 13 domains and 47 classes related to nursing practice.
Problem-Focused Nursing Diagnosis
A nursing diagnosis representing a client problem present at the time of assessment, based on signs and symptoms.
Risk Nursing Diagnosis
Clinical judgments indicating a problem does not exist but risk factors suggest a potential problem may develop.
Health Promotion Diagnosis
A clinical judgment about motivation and desire to increase well-being, moving from one level of wellness to a higher level.
Syndrome Diagnosis
A judgment concerning a cluster of related nursing diagnoses due to a specific situation or trauma.
Defining Characteristics
Clusters of signs and symptoms that indicate the presence of a specific nursing diagnosis.
Aetiology
Identifies one or more probable causes of a health problem in a nursing diagnosis.
Risk Factors
Conditions or forces that increase susceptibility to an unhealthy condition, used instead of aetiological factors in risk diagnoses.
