(3,4,6 together) Check pupil size, pupil constrict with light, consensual (other pupil constricts), whiskers (accommodation), convergence (together when pen is moved to eyes), Pupils Equal Round Reactive to Light and Accommodation (PERRLA)

4. Trochlear

(3,4,6 together) Check pupil size, pupil constrict with light, consensual (other pupil constricts), whiskers (accommodation), convergence (together when pen is moved to eyes), Pupils Equal Round Reactive to Light and Accommodation (PERRLA)

5. Trigeminal

Bite (motor) Cotton on skin (sensory)

6. Abducens

7. Facial

Smiling, puffing cheeks, and raising eyebrows.

8. Acoustic

Whisper test

9. Glossopharyngeal

Gag reflex.

10. Vagus

Saying 'ah,' checking uvula midline, swallow.

11. Spinal Accessory

Shoulder shrug, turn head side to side

12. Hypoglossal

Tongue movement from side to side.
