Drowning
Suffocation and death as a result of submersion in liquid.
Dry Drowning
The glottis spasms and prevents water from passing into the lungs.
Wet Drowning
Glottis relaxes and allows water to flood the tracheobronchial tree and alveoli
Near Drowning
Situation in which a victim survives a liquid submersion, at least temporarily.
Primary Assessment findings in Near Drowning
Increased HR, RR, BP, May have apnea
Inspection findings for Near Drowning
Cyanosis, frothy secretions
Palpation findings for Near Drowning
Increased fremitus and decreased expansion
Percussion findings for Near Drowning
Hypo-resonance if atelectasis or consolidation develops
Auscultation findings for Near Drowning
Diminished, crackles
ABG findings for Near Drowning
Acute Ventilatory Failure with Hypoxemia
CXR findings for Near Drowning
Fluffy infiltrates
Management of Near Drowning
CPR if needed, Intubation/mechanical ventilation
Etiology of Near Drowning
Restrictive
Anatomic Alterations in Near Drowning
Laryngospasm, Interstitial edema, Decreased pulmonary surfactant with increased surface tension of alveolar fluid
More Anatomic Alterations in Near Drowning
Frothy, white, and pink secretions, Atelectasis, Alveolar consolidation, Bronchospasm
Favorable factors in near drowning
Younger age, short submersion time, colder water, clean water, no other serious injuries, less struggle, good CPR
