Excision
Cutting out or off without replacement a portion of a body part(with sharp tools)
Ex: biopsy, cyst, lesions
Resection
Cutting out or off, without replacement, all of a body part
Ex: lungs, lobes, intestines
Detachment
Cutting off all or a portion of the upper or lower extremities
Ex: amputations, forequarter, hindquarter, rays, complete, partial
Destruction
Physical eradication of all or a portion of a body part by the direct use of energy force or a destructive agent. No biopsy.
Ex:ablation, coblation, cryosurgery, electrosurgery, coagulation.
Extraction
Pulling or stripping out or off all or a portion of a body part with force(no cutting)
ex: suction, tooth, stripping veins, dilation and curettage, bone marrow, and endometrial biopsies, nonexcisional debridement.
Drainage
Taking or letting out fluids and/or gases from a body part
ex: thoracentesis
Extirpation
Taking or cutting out solid matter from a body part.
Ex: thromectomy, foreign body,
Fragmentation
Breaking solid matter into pieces(not taking out)
Ex: Shockwave, lithotripsy, ultrasound
Division
Cutting into a body part without draining to separate or transect a body part(separate, cut)
Ex: rhizotomy
Release
Freeing a body part from an abnormal physical constraint by cutting or force.(freeing)
ex: carpal tunnel release
Transplantation
putting in or on all or a portion of a living body part taken from another individual or animal to physically take the place and/or function of all or a portion of a similar body part. (no cornea, heart valve, or bone marrow)
ex: heart, lungs
Reattachment
Putting back a part that is cut off completely
ex: avulsed kidney, hand
Transfer
Moving without taking out all or a portion of a body part to another location to take over the function of all or a portion of a body part.(from and to is coded). keeps blood and nerve supply
Ex:pedicle, flaps, and NOT free grafts.
Reposition
Moving to its normal location or other suitable location all or a portion of a body part.
Ex: ONLY displaced fracture or undescended testicles, osteotomy
Restriction
Partially closing an orifice or lumen of a tubular body part( can include intraluminal or extraluminal devices)
ex: clipping of cerebral aneurysm
Occlusion
Completely closing off an orifice or lumen of a tubular body part
ex: ligation of fallopian tubes or tying off hemmorhoids, embolization
Dilation
Expanding an orifice or the lumen of a tubular body part(can include intraluminal or extraluminal devices)
Ex: Percutaneous Transluminal Coronary Angioplasty (P T C A)
Bypass
Altering the route of passage of the contents of a tubular body part
noncoronary: body part and qulaifier is “from and to”
coronary: body part and qualifier is “number of arteries bypassed and from”
Device: graft material,
Ex: CABG, Colonstomy
Insertion
Putting in a nonbiological device that monitors, assists, performs, or prevents a physiological function but does not physically take the place of a body part
Ex: Neurostimulator, bone growth stimulator, cardiac pacemaker
Removal
Taking out or off a device from a body part
ex: removing drainage or internal fixation device
Revision
Correcting, to the extent possible, a portion of a malfunctioning or the position of a displaced device
Change
Taking out or off a device from a body part and putting back an identical or similar device in or on the same body part without cutting or puncturing the skin or a mucous membrane.
ex: exchange, putting back same or similar device
Supplement
Putting in or on biologic or synthetic material that physically reinforces and/or augments the function of a body part
ex: hernia repair using a mesh, exchange tube or catheter
Replacement
Putting in or on biological or synthetic material that physically takes the place and/or function of all or a portion of a body part
ex:hip replacement, free skin graft, cataract removal
Inspection
Visually and/or manually exploring a body part( instruments may be used)
ex: bronchoscopy, arthrotomy, colonoscopy
Map
Locating the route of passage of electrical impulses and/or locating functional areas in a body part( only used for central nervous system and cardiac conduction system)
ex: brain mapping, cardiac mapping
Repair
Restoring, to the extent possible, a body part to its normal anatomic structure and function( only used if no other root operation applies)
-NEC: not elsewhere classified
ex: suture of lacerations, herniorraphy
Control
Stopping, or attempting to stop, postprocedural or other acute bleeding
-only coded to ear nose sinus, general, lower, and upper extremities
Fusion
Joining together portions of an articular body part rendering the articular body parts immobile
only in joints and based on number of joints not number of vertebrae
ex: spinal fusion
Alteration
Modifying the natural anatomic structure of a body part without affecting the function of the body part( mostly cosmetic can be medical)
ex: breast reduction, blepharoplasty
Creation
Putting in or on biological or synthetic material to form a new body part that to the extent possible replicates the anatomic structure or function of an absent body part
ex: heart valves or gender reassignment
