Objective Assessment
A method used to observe and measure physical findings, including recognizing and acting upon abnormal cues.
Trauma-informed Approach
An approach that considers the impact of trauma on individuals and incorporates physical touch in assessments.
Hand Hygiene
The most critical action in infection prevention and control, aimed at reducing the transmission of microorganisms.
Anatomical Position
A standard position of the body used as a reference point for anatomical location.
Inspection
A purposeful and systematic examination method that involves bilateral comparison and assessment of the client overall and in specific body areas.
Palpation
An assessment technique that involves using hands or fingers to examine the body, noting normal and abnormal findings.
Temperature (in palpation)
Refers to the degree of heat or cold of a surface, often assessed using the dorsal aspect of the hands.
Texture
The smoothness or roughness of a surface as assessed through palpation.
Crepitation
An abnormal grating or crunching sensation felt and heard over joints where bones meet.
Auscultation
The process of listening to body sounds using a stethoscope, assessing both high-pitched and low-pitched sounds.
Percussion
A technique involving tapping the body to elicit sounds that help determine the consistency of underlying structures.
Tympany
A sound indicating that an abdominal area, such as the intestines and stomach, is filled with air.
