acetaminophen
MOA - activates descending serotonergic inhibitory pathway to CNS
Indication - mild to moderate pain or fever, HA
Contraindication - hypersensitivity, aspartame, saccharin
Parameters - don’t exceed 4mg per day, monitor AST & ALT, watch for jaundice
SE - report signs of dark urine, yellowing of the eyes, or persistent nausea
Digoxin
MOA - inhibits NA/K ATPase making calcium more available for contractile proteins
Indications - A-fib, HF
Contraindications - V-fin, V-tach, 2nd or 3rd degree heart block
Parameters - hold for HR <60 bpm, monitor for bradycardia and dysrhythmias
SE - HA, yellow-green halos, drowsiness
Heparin
MOA - prevents conversion of fibrinogen to fibrin
Indications - prevention and treatment of MI & treatment of DIC
Contraindications - active bleeding, TCP, severe, uncontrolled HTN
Parameters - monitor aPTT and PTT, watch for HIT ans signs of bleeding
SE - more frequent nose bleeds, bleeding gums (use soft bristle toothbrush)
Influenza
MOA - stimulates immune system to produce antibodies against influenza virus
Indications - prevention of seasonal influenza
Contraindications - allergy to vaccine components, moderate to severe acute illness
Parameters monitor for signs of allergic reaction after injection
SE - soreness at injection site, mild fever, fatigue
Regular Insulin
MOA - decreased blood glucose by increasing transport of glucose into cells and increases the conversion of glucose to glycogen
Indications - Type 1 and 2 DM, GD
Contraindications - hypoglycemia, renal impairment, liver disease
Parameters - Check blood glucose before, watch for signs of hypoglycemia (shaky, sweaty, tachycardia)
SE - tachycardia, muscle weakness, and fatigue
Lisinopril
MOA - suppresses RAAS, prevents conversion of angiotensin I to angiotensin II
Indications - mild to moderate HTN, acute MI
Contraindications - Hx of angioedema, hyperkalemia, bilateral renal artery stenosis
Parameters - monitor for low BP, monitor BUN/creatinine
SE - dry persistent cough, muscle weakness, fatigue
