Ask directly, create safe environment, close observation, med admin watch, make irregular interval rounds, encourage honest feelings. Establish trust, collaborate with pt for safety plan. asses verbal and non verbal clues.

Take any hint of suicide seriously, do not keep secrets, be a good listener, express to the client feelings or personal worth, know about suicide intervention resources, restrist access to firearms or other way to self harm.
