ACLS Algorithms

Adult Cardiac Arrest Circular Algorithm

1. Start CPR

   1. Give Oxygen

   2. Attach Monitor / Defibrillator

2. Check Rhythm

   1. If VF/VT Shock

3. Continuous CPR for 2 Minutes

   1. IV/IO Access

   2. Epi every 3-5 minutes

   3. Amiodarone/Lidocaine for refractory VF/VT

   4. Treat Reversable Causes

   5. Return to step 2.
       

Adult Cardiac Arrest Algorithm - VF/VT

1. Shock

2. CPR 2 minutes

   1. IV/IO Access

3. Shock

4. CPR 2 minutes

   1. Epi every 3-5 minutes

   2. Consider Advanced Airway

   3. Capnography

5. Shock

6. CPR 2 Minutes

   1. Amiodarone or Lidocaine

   2. Reversable Causes

7. Repeat

Adult Cardiac Arrest Algorithm - Asystole/PEA

1. CPR 2 minutes

   1. IV/IO Access

   2. Epi q3-5 minutes

   3. Airway and Capnograhy

   4. Treat Reversable Causes

Adault Post-Cardiac Arrest Care Algorithm - ROSC

1. Confirm ROSC

   1. Manage Airway

   2. 10 breaths/min

   3. SpO2 92%-98%

   4. Keep SBP over 90 and MAP over 65

2. Obtain 12-Lead

   1. Check for STEMI

   2. Caridogenic Shock

   3. Mechanical Circulatory Support needed?

3. Check for Mentation / Able to follow commands?

   1. No

      1. Target temperature management

      2. Brain CT

      3. EEG monitoring

      4. Other Critical Care management

   2. Yes

      1. Other critical care management

4. Eval and Treat rapidly

Adult Bradycardia Algorithm

1. Initial

   1. Maintain Airway

   2. Oxygen

   3. Cardiac Monitor and BP

   4. IV access

   5. 12-Lead if able

2. Persistent Bradycardia

   1. No

      1. Monitor and Observe

   2. Yes

      1. Signs of Shock

      2. Atropine 1mg

      3. Transcutaneous Pacing

      4. Dopamine 5-20mcg

      5. Epi push dose 2-10mcg

Adult Tachycardia Algorithm

1. Initial

   1. Maintain Airway

   2. Oxygen

   3. Cardiac Monitor and BP

   4. IV access

   5. 12-Lead if able

2. If Signs of Shock, Synch Cardiovert

3. If QRS wide, consider amiodarone 150mg over 10mins

4. If narrow QRS,Amiodarone if SVT, or cardizem
