Petechiae
Small red or purple pinpoint hemorrhages under the skin; flat and non-blanching, often seen on skin or mucous membranes.
Mydriasis
Dilation of the pupil (bilateral) usually in response to low light or sympathetic stimulation.
Miosis
Constriction of the pupil (bilateral) usually in response to light or parasympathetic stimulation.
Wound staging
A system to categorize wound depth and severity (e.g., stages I–IV); used to guide treatment and documentation.
Kilograms to pounds (kg to lb)
Multiply kilograms by 2.2046 to convert to pounds.
Pounds to kilograms (lb to kg)
Divide pounds by 2.2046 (or multiply by 0.4536) to convert to kilograms.
Pill counting / percentage remaining
Calculate how many pills have been taken or remain by percentage: remaining = (pills left/total pills) × 100; taken = total − left.
Cherry hemangioma
Small, bright red, benign vascular skin lesion common in adults; usually appears with aging.
Chart by exception
Documentation approach where only abnormal findings are recorded; normal findings are assumed.
Snellen chart
Eye chart used to measure visual acuity; patient stands 20 feet away and reads lines from top to bottom, one eye at a time.
Snellen chart directions
Instruct the patient to cover one eye, read the smallest line possible, then repeat with the other eye and with both eyes.
Nystagmus
Involuntary, rapid, rhythmic eye movements; can indicate neurologic or vestibular dysfunction.
Closed head injury symptoms
Common symptoms include headache, confusion, dizziness, vomiting, loss of consciousness; monitor for changes.
Range of motion (ROM) assessment
Assessment of joint movement through its full range to evaluate mobility, especially for large joints (shoulders, hips, knees, elbows).
