- It isn't possible for prescribing MD to provide written rx to RPh before dispensing

- Patient is enrolled in hospice care program certified/paid by Medicare

- Terminally ill patients (ex. hospice)

- LTCF patients (partial fills allowed for up to 60 days from date of issuance)

- If patient requests partial fill (may still pick up remainder before 30 days of issuance is up)

- Partial filling is recorded in the same manner as a refill (date refilled, amt dispensed, initials of dispensing RPh, etc.)

- Total quantity dispensed doesn't exceed total quantity prescribed

- No dispensing after 6 MONTHS past date of issue

- All scripts must be dated with the date of issuance (date written)

- Subsequent scripts must contain a "Do not dispense before ___ (date)"
