What are the 5 general approaches to Airway Clearance Therapy (ACT)?
Chest physical therapy (CPT) 2. Cough (directed, huff) 3. PAP adjuncts (handheld devices) 4. High frequency compression (IPPB) 5. Mobilization/physical activity
What is the most effective type of Airway Clearance Therapy?
Mobilization/physical activity
What are the three requirements for normal airway clearance?
Patent airway 2. Functional mucociliary escalator 3. Effective cough
What can full obstruction from retained secretions lead to?
Atelectasis
What can partial obstruction from retained secretions lead to?
Increased work of breathing, air trapping, V/Q imbalances
What are 6 findings that indicate a need for ACT?
Loose, ineffective cough 2. Labored breathing pattern 3. Coarse inspiratory and expiratory crackles 4. Tachypnea/tachycardia 5. Fever 6. Chest radiography demonstrating atelectasis and infiltrates
What are the 3 different types of Chest Physical Therapy (CPT)?
Position/gravity 2. Cupping 3. Vibration
What is the Forced Expiratory Technique (Huff Cough)?
A modification of directed cough that consists of one or two forced expirations of middle to low lung volume without closing the glottis
What is the goal of a huff cough?
To clear secretion with less change in pleural pressure and less bronchial collapse
What is a Mechanical Insufflation-Exsufflation (MIE) device?
A cough assist device that applies positive pressure then abruptly reverses airway pressure to assist patient with cough
What are the indication for CPT?
CF, bronchiectasis, lobar atelectasis from mucus plugging
What are the indications for PEP therapy?
CF, COPD, atelectasis prevention
What are the indications for High Frequency Chest Wall Oscillation
CF, Neuromuscular disease patients
What are the indications for Intrapulmonary Percussive Ventilation (IPV)
CF, COPD, post-op patients
What are the indications for directed cough and Huff cough?
Nearly all patients with weak or ineffective cough
What are the indications for mechanical insufflation-exuffraltion?
Neuromuscular disorders, patients unable to generate sufficient expiratory force
