Identify Problem
The problem may only be a symptom of the real problem. You need to ask questions to be able to solve the actual problem. Observe, pay attention, and confront difficult issues and look beyond the obvious.
Gather Information
Information needs to be accurate and reliable and need to separate emotions from facts. Objective data can be collected and consists of what is measured (signs) while subjective data is the feelings and what is reported by the patient (symptoms).
Create Alternative Solutions
Generate all and every possible solution to the problem. Consider the negatives and positives of each solution. Discard the ones that are impractical.
Choose Alternative + Take Action
Come up with the solution and follow through
Evaluate and Revise as Needed
Evaluate the results of the actions taken and adjust accordingly if need be as circumstances can change.
