Upper fibers elevate and upwardly rotate, middle fibers adduct, and lower fibers depress scapula

Spinal accessory nerve (CN 11); anterior rami C3 and C4

Rhomboid major proximal attachment (origin)

Spinous processes of T2- T5 vertebrae

Rhomboid major distal attachment

Lumborum, thoracis, cervicis; fibers run superiorly to angles of lower ribs and cervical transverse processes

Iliocostalis action

Bilateral - extend the vertebral column and head; as back is flexed, control movement via eccentric contraction Unilateral - laterally flex vertebral column to the same side

Longissimus proximal attachment (origin)

Arises by a broad tendon from posterior part of iliac crest, posterior surface of sacrum, sacroiliac ligaments, sacral and inferior lumbar spinous processes, and supraspinous ligament

Longissimus distal attachment

Thoracis, cervicis, capitis; fibers run superiorly to ribs between tubercles and angles to transverse processes in thoracic and cervical regions, and to mastoid process of temporal bone

Longissimus action

Bilateral - extend the vertebral column and head; as back is flexed, control movement via eccentric contraction Unilateral - laterally flex vertebral column to the same side

Aponeurosis of the erector spinae muscles, sacrum and transverse processes of lumbar, thoracic and cervical vertebrae

Multifidus distal attachment

Spinous processes of lumbar, thoracic, and lower cervical vertebrae, spans 2-4 levels

Multifidus action

Extend the vertebral column and rotates the vertebral column contralaterally Multifidus has a lumbar stabilization component

Segmentally innervated by dorsal primary rami of spinal nn.
