Differential Diagnosis for Tension vs. Migraine vs. Sinus HAs

Tension: Bilateral, diffuse pain over top of head or at rear base of skill. Radiating, “tight pressing pain” w/ gradual onset. “Stress HA”

• Typically respond well to OTC analgesics.

Migraine: Recurrent hemi-cranial throbbing pain lasting hours to days. Often associated with N/V, photo/phonophobia, aura, etc.

• Dr diagnosis required for treatment. (better Rx options vs. OTC), Avoid dietary triggers

Sinus: Localized to forehead/periorbital areas. Pain worse upon blowing nose. Persistent pain w/ discharge may suggest infection.

• OTC analgesics/decongestants may relieve pain.

Pseudoephedrine Sale Restrictions

Combat meth production - keep log of sales for 3 years.

• 2 Boxes/Transactions - NO MORE than 2 g/package

• 3.6 g/day MAX - 9 g/month MAX

Must be 18 & have valid govt. ID

Headache: Self-Care Exclusions

• Severe head pain

• Headaches persisting for 10 days with or without treatment

• Last trimester of pregnancy

• <8 years of age

• High fever or signs of serious infection

• History of liver disease or consumption of >=3 drinks/day

• Secondary Headache

• Symptoms consistent w/ Migraine, but no formal diagnosis.

Fever: Self-Care Exclusions

• Patients >6 mo. w/ rectal temperature =>104 or equiv.

• Children <6 mo. w/ rectal temp. =>101

• Symptoms of Infection

• Impaired O2 utilization, respiratory distress, HF

• Immparied immune function

• CNS damage

• History of seizures

• Fever persists >3 days

• Childs w/ spots/rash, refusal of fluid intake, sleepy/irritable/hard to wake, vomiting, not keeping down fluids
