A single provider group working within a defined scope
Multidisciplinary
Providers from different disciplines and perspectives working independently, parallel, or sequentially
Interdisciplinary
Providers working together through a discipline specific basis, teams have one or more leaders
Interprofessional
Providers combine unique knowledge and purposely interact to work and learn together, and with patients, to achieve a shared goal
Transdisciplinary
Professionals from multiple disciplines where professional boundaries are blurred, and perspective transcend each other to form a new holistic approach
