Skill IV: Compounding an Oral Suspension (15 minutes)

obtain completed recipe/formula using Master Formula sheet

check amount per dose and # of doses needed with judge

use Master Formula sheet to obtain the correct medication & the correct # of tablets; verify with judge

obtain needed equipment (mortar & pestle, graduated cylinder, suspending vehicle)

wash hands OR use alcohol based hand-rub

• don PPE in the following order: 

  • Gown 

  • Hair cover

  • Face mask

  • Gloves (ensure covers cuffs of gown)

crush tablets into fine powder with mortar & pestle

add small amount of suspending vehicle to wet powder, then mix/levigate powder until smooth paste is formed

continue to add suspending vehicle until product can be poured into graduated cylinder

pour liquid into graduated cylinder

rinse mortar with small amounts of suspending vehicle and add to graduated cylinder

add suspending vehicle until product is at required volume, verify bottom of meniscus is at required volume at eye level

pour compounded suspension into most appropriately/closely sized amber oval medication bottle, close and shake well

select appropriate label and stick onto medication bottle; verify with judge

select appropriate auxiliary label and stick onto medication bottle ; verify with judge

16

clean area and equipment using 70% isopropyl alcohol, return equipment to proper place

17

remove and dispose of PPE

wash hands or use alcohol based hand-rub
