Have you ever had a TB test?

¿El/la paciente ha tenido una prueba de tuberculosis?

What is the date of your last tuberculosis test?

¿Cuál es la fecha de su ultima prueba de tuberculosis?

What was the result? Positive or negative?

¿Cuál fue el resultado? ¿Positivo o negativo?

Did you get a chest x-ray?

¿Le tomó rayos-x?

Have you ever been treated for tuberculosis? When? What medication? For how long?

¿Alguna vez ha recibido tratamiento para la tuberculosis? ¿Cuándo? ¿Cuál medicina? ¿Por cuánto tiempo la tomó?

Have you ever had: chronic cough, coughing up blood, unexplained fever, weight loss, or sweating at night?

¿Ha tenido: tos cronica, sangre al tocer, fiebre inexplicable, perdidad de peso, o suda mucho en la noche?

Have you had close contact with anyone who is sick with tuberculosis? (for example: coughing, weight loss, etc.)

¿Ha tenido contacto cercano con alguien que este enfermo de tuberculosis? (por ejemplo: mucha tos, perdida de peso, etc.)

Have you lived in Asia, Mexico, Latin America, or the Middle East in the past 3-5 years?

Alternative: ¿ha vivido en un otro país en los pasados 3-5 años?

Do you spend significant time in a household with anyone who has: HIV/AIDS, history of long term drug use, or someone who was released from prison or arrived from another country in the past year?
