SBAR

Situation

Background
The second component of SBAR; includes admission diagnosis, date, pertinent medical history, and treatment history.
Assessment
The third component of SBAR; includes vital signs, 02 requirements, and changes from previous assessments or baseline.
Recommendation
The fourth component of SBAR; outlines the actions requested or recommended.
MD
Medical Doctor; a healthcare professional involved in patient treatment.
Interdisciplinary Team
A group of healthcare professionals from various fields working together for patient care.
Hematocrit
The volume percentage of red blood cells in blood; a measure used to assess anemia or dehydration.
Vital Signs
Clinical measurements that indicate the state of a patient's essential body functions.
Patient transfer
