what is (Long Term Care)?
given to people who need 24 hour skilled care
what is (skilled care)?
-medically necessary care given by a skilled nurse or therapist
-ordered by doctors and involve treatment plans
Formerly called (Nursing Homes)
they are now called Long Term Care facilities, skilled nursing facilities, rehabilitation centers and extended care facilities.
whats is (length of stay)
the number of days a person stay in the facility
what is (terminal illness)
this means that the illness will eventually cause death
what is (chronic conditions)
conditions that last a long period of time even a lifetime
what is (residents)
-people who live in long-term facilities
-different diagnosis means different type of care needed
what is (home health care)
-provided to a persons home
-people who are able to stay and wish to stay home
what is (assisted living)
-facilities that help with daily task “showering, eating etc”
-people who do not need 24 hour care
-allow more independent living in home like environment
-an assisted living facility may be attached to a long term facility
what is (adult day services)
-people who need some help and supervision during certain hours
-for people who need some help but not seriously ill or disabled
what is (acute care)
-24 hour care given in hospitals and ambulatory surgical centers.
-it is for people who require short term, immediate care for illnesses and injuries
-also short days for surgery
what is (subacute care)
-given in hospitals
-use for people who need less care than acute
-treatment ends when condition is stable
-more care than chronic
-cost is less than acute care but more than longterm care
outpatient care
-given to people who had treatments or surgeries and need short term skilled care
-do not require overnight stay in the hospitals or other care facilities
rehabilitation center
-given by a specialist
-help restore function after illness or injury
hospice care
-given to facilities or homes of people who have 6 months left to live
-provide emotional and physical care until death
-support families in this process

-are businesses that provide skilled nursing care 24 hours a day
-offered assisted living housing, dementia care or subacute care
