IVT
● Introduction of catheter to the vein of the patient

Isotonic solutions
solutions have equal solute concentrations, causing no net water movement.
Hypotonic solutions
solutions have a lower solute concentration, causing water to move in and cells to swell
Hypertonic
solutions have a higher solute concentration, causing water to move out and cells to shrink.

Isotonic Solution
-check the doctor's order
-Communicate with the patient first before spiking
-Prepare the materials
-spiking
-remove air bubble
-go to patient
-apply tourniquet
-insert the needle

-regulate IV
-change IV solution if 50 ml
Initiating IV
● Check the doctor's order
● Stop the infusion
● Add alcohol to the cotton
Discontinuing IVT
BOLUS
● Slow IV push ● 30 minutes
INFUSION
● Antibiotics are usually in soluset ○ Vancomycin ● Usually 30 mins - 1 hour
TANDEM SET-UP
● 2nd container - attached to the line of the first container at the lower port ● Medications are administered intermittently or simultaneously with the primary solution
PIGGY BACK
● The 2nd set connects the second container to the tubing of the primary container at the upper port ● Used solely for intermittent during administration
Phlebitis
○ Continuous puncturing of the vein ○ Infection may happen
INFILTRATION
IV fluid leaks into surrounding tissue
(IV was not placed properly in the vein)
PHLEBITIS
Inflammation of the vein.
Can lead to a clot (thrombophlebitis)
HEMATOMA
Collection of blood in the tissues
(typically happens when you "blow the vein")
Infection
Entry of microorganism into the body via IV
● Check the physician’s order
● Check indication
● Assess the patient
● Insert IV line using PNSS Solution
● Administer pre-blood transfusion medications as orders
● Transfuse the blood within prescribed time
● Monitor the patient accordingly ○ First hour, every 15 minutes
● Monitor for BT Reactions ○ Hemolytic reaction ○ Anaphylactic reaction
● Document the procedure one
BLOOD TRANSFUSION NC
● Check the physician's order
● Check indication
● Assess the patient
● Check the IV Site (Central lines/PICC) ○ Ideally it is central line
● Administer the solution for 24 hours
● Do not catch up if delayed in the transfusion
● Monitor the patient accordingly
● Monitor blood glucose level through CBG
● Document the procedure done
