Gathering information from the patient through subjective and objective data

Diagnose

Once info has been gathered, identify the priority problem to create a nursing diagonsis.

Plan

Set goals to solve the patient’s problem; What am i gonna do to fix this problem?

Implement

Begin nursing interventions (eg: elevating the pt’s bed, adminstering medication).

Basically, the nurse is putting the care plan into action

Evaluate

Determine outcome of goals and actions. Did they work to solve the pts problem? What needs to be changed?

Objective data

Information that can be directly observed and measured by the nurse (eg: a wound, blood pressure reading, EKG, Vital signs)

Subjective data

Subject to change. Info that the pt or family tells the nurse. (eg: pt reports pain of 6/10)

Symptoms that cannot be directly observed or measured, such as a patient’s pain level.
