23 year old presents with weakness of increasing severity for the past 2 days with dysphagia. Has not had any urine output for 24 hours despite copious fluid intake and a palpable bladder to the periumbilical region. Initially patient had some motor function in the lower extremities but now appears completely paralyzed with decreased DTR's. Pertinent hx includes ingestion of home canned
green beans 3 days prior. On admission patient goes into respiratory arrest and is put on a ventilator
Clostridium botulinum
14 year old presents with nausea, vomiting, diarrhea, low grade fever and abdominal pain for the past 3 days. Last meal prior to symptom onset was a fried egg sandwich his Mom made for him. He is sad as he is missing show and tell and wanted to bring Ringo his pet turtle. On PE he has mild diffuse abdominal tenderness and signs of dehydration.

A 28 year old male Peace Corp volunteer home for about 3 weeks complains of a gradual onset of intermittent crampy abdominal pain with foul smelling frothy loose stools daily. The stools sometimes contain blood and mucus. He also complains of lower abdominal tenderness, flatulence and tenesmus. He thinks he may have lost some weight as well. On PE he has tenderness in the RUQ and a positive Murphy's sign. Slight hepatomegaly is noted.
