Population health requires OTP to adopt or expand a systems thinking perspective

Vision statement:

ideal state or ultimate level of achievement to which an organization aspires

• long term, aspirational, inspire or motivate, describes a future state

Mission statement:

an organization’s core underlying purpose or basis for its existence, focus, and actions

• present day, defines the core purpose, clarification on how to reach a vision, outlies what the organization does

What is an implementation plan?

Includes the goals, objectives, activities, and desired outcomes of the program

• who will be served by the program?

• how the individuals being served will be recruited?

* focus on details of who, what, when, where, and how for the program

Program goal:

a statement of quantifiable desired future state or conditions

• how can the identified community need be addressed?

Program objective:

measurable, short term, and usually contains a time for completion

• what can be done to meet the goals of the program?

indicate the programs priorities and intended outcomes

Location and space:

what you need to consider

• liability

• accessibility (building, transportation)

• regulations

• use of space

• space maintenance

Staffing and Personnel:

what you need to consider

• job descriptions

• staff evaluation and promotion

• volunteer roles

  • define roles, access resources (university students, corporate volunteer hour benefit)
