HIV infection
Breastfeeding is completely contraindicated in HIV-positive mothers in the U.S.
Untreated Active Tuberculosis
Breastfeeding is contraindicated until the mother has been treated for at least 2 weeks and is non-infectious.
Active Varicella Infection
Breastfeeding is contraindicated if the mother develops a rash within 5 days before or 2 days after delivery.
Herpetic Breast Lesions
Localized lesions on the breast lead to stopping breastfeeding from the affected side only.
HTLV-1 or HTLV-2 infection
HTLV-positive mothers should not breastfeed.
Illicit drug use
Active use of substances like cocaine, PCP, or heroin contraindicates breastfeeding.
Maternal Febrile Illness
Breastfeeding can continue with precautions during maternal febrile illnesses like influenza.
Radiation Therapy
Breastfeeding should be paused while the mother is undergoing active chemotherapy or radiation therapy.
Galactosemia
Breastfeeding is contraindicated in newborns with galactosemia; a soy-based formula is required.
PKU (Phenylketonuria)
Partial breastfeeding may be allowed with special monitoring for infants with PKU.
Hepatitis B
Breastfeeding is allowed if the infant is vaccinated and the mother's nipples are not cracked.
Hepatitis C
Breastfeeding is generally allowed unless the mother has cracked or bleeding nipples.
Methadone-maintained mothers
Mothers who are stable on methadone maintenance therapy can breastfeed.
Temporary Hold on Breastfeeding
Used for specific cases like maternal chemotherapy, radiation, and certain medications.
