Anatomic Areas
Aortic, Pulmonic, Tricuspin, and Mitral Areas.
Rough Z pattern.
Auscultation
Over anatomic areas: heart rate is regular with normal S1 and S2. No extra heart sounds or murmurs.
Repeat with bell: no extra heart sounds or murmurs heard with bell.
Change Positions
Apex (5th ICS, left midclavicular line), with person left lateral using bell.
Base (2nd ICS, right sternal border), with person sitting, leaning slightly forward and exhaling using diaphragm.
No extra heart sounds or murmurs detected in left lateral/sitting positions.
Apical and Radial Pulses
Auscultate at apex and palpate radial pulse.
Apical and radial pulses are equal and synchronous.
