1. Request for an evaluation - prior written notice for initial evaluation and request for consent form

2. nondiscriminatory evaluation (NE or NDE) - evaluation report written

3. Multidisciplinary team meeting (MDT) is held

4. Individualized Education Program (IEP) meeting is scheduled - invitation to participate in the IEP team meeting or other meeting

5. the IEP meeting is held, an Indivudalized Education Program (IEP) is written
