Acute Care Hospital
Comprehensive medical management for acute conditions or exacerbation of chronic conditions.
Rehabilitation Facility
Provides complex medical management or intensive rehabilitation services; patient must be able to tolerate and benefit from a minimum of 3 hours of therapy per day.
Skilled Nurse Facility (SNF)
Provides skilled nursing and rehabilitative services; also referred to as an extended care facility
Nursing Home
Provides unskilled care; patients may receive a functional maintenance program carried out by certified nursing aides; Medicare NOR insurance pays
Hospice Care
Covered by insurance and Medicaid; covered under Medicare if patient has been deemed with less than 6 months to live; includes intermittent nursing, PT, OT (if at home), home health aide and homemaker, and drugs including outpatient drugs.
Home Health (Medicare Coverage Conditions)
Needs intermittent skilled nursing, is homebound, and certified there is a reasonable expectation that significant functional improvement will result from treatment

Set standards for nursing homes, including 24-hour care.
