Several times throughout the exam and again when finished.

Resume previous exam, faculty reviews, click Complete, faculty swipes for approval.
How many probing depths are recorded per tooth?
Six per tooth.
What does probing depth (PD) measure?
Distance from the free gingival margin (FGM) to the bottom of the sulcus or pocket.
What instrument is commonly used to measure PD?
UNC-15 periodontal probe.
How is bleeding on probing recorded?
Click “B” for the site that bleeds.
When is bleeding recorded?
If bleeding is immediate or delayed.
How is mobility recorded in Axium?
Using Arabic numerals (0,1,2,3).
Mobility 0
No mobility (within normal limits).
Mobility 1
Up to 1 mm buccolingual movement.
Mobility 2
Up to 2 mm buccolingual movement, not depressible.
Mobility 3
Greater than 2 mm movement and/or depressible.
Must mobility be recorded on all teeth?
Yes, even if it is 0.
What does FGM stand for?
Free Gingival Margin.
What does FGM measurement represent?
Distance from FGM to CEJ.
What is the CEJ?
Cementoenamel junction (where enamel meets cementum).
When is FGM recorded as a positive number?
When FGM is coronal (above) the CEJ.
When is FGM recorded as a negative number?
When FGM is apical (below) the CEJ, indicating recession.
How many FGM measurements are recorded per tooth?
Six per tooth.
If no FGM is entered in Axium, what does the system assume?
It assumes FGM equals 0.
Why is not entering FGM a problem?
It causes incorrect CAL calculations.
Formula for Clinical Attachment Level (CAL)
FGM + PD = CAL.
What does CAL measure?
True attachment loss from CEJ to the bottom of the pocket.
Example: PD 3mm and FGM 3mm
CAL = 0 (no attachment loss).
Example: PD 3mm and FGM 2mm
CAL = 1mm attachment loss.
Example: PD 2mm and FGM -5mm
CAL = 7mm attachment loss.
What happens if FGM is incorrectly recorded as 0 everywhere?
Axium falsely shows attachment loss.
What does the blue line represent in Axium?
The position or contour of the gingival margin.
How is furcation involvement recorded?
Using Arabic numerals (0,1,2,3).
Furcation Class 1
Slight or beginning involvement.
Furcation Class 2
Moderate involvement, not through-and-through.
Furcation Class 3
Through-and-through involvement.
What does “K” on the chart indicate?
Inadequate keratinized gingiva (less than 2 mm).
How is suppuration displayed?
Yellow triangle.
How is an overhang displayed?
Gray semilunar shape.
What must a complete periodontal chart include?
PD, FGM, CAL, BOP, mobility, furcation, plaque, calculus, and other conditions.
