Nursing Process
A systematic series of actions that includes assessment, nursing diagnosis, planning, implementation, and evaluation.
Assessment
The first step in the nursing process that involves comprehensive data collection, including physical examination, nursing history, and medication history.
Nursing Diagnosis
A clinical judgment about individual, family, or community responses to actual or potential health problems/life processes.
Four Types of Nursing Diagnosis
Collaborative Problems
Different from nursing diagnoses, these involve interventions to prevent or treat a problem and are worded with potential complications.
Evidence-Based Practice
Applying scientific research data to make clinical decisions to improve patient outcomes.
Measurable Goal Statement
A goal that starts with an action word and outlines behaviors to be performed by the patient or family within a specific timeframe.
Nursing Intervention or Implementation
The process of executing a plan of care through dependent, interdependent, or independent actions.
Dependent Nursing Actions
Actions performed by a nurse based on healthcare provider’s orders.
Independent Nursing Actions
Actions not prescribed by a healthcare provider that a nurse can perform based on education and licensure.
Therapeutic Outcomes
Statements developed to evaluate the effectiveness of the care provided.
Evaluation
The final phase of the nursing process where the nurse determines if expected outcomes were met.
Primary Source
Information obtained directly from the patient.
Risk Diagnosis
A clinical judgment that a person is more susceptible to a particular problem than others in the same situation.
Health Promotion Diagnosis
A one-part nursing diagnosis focused on patients' readiness to improve their health.
Maslow's Hierarchy of Needs
A framework for prioritizing patient care needs starting with physiological needs up to self-actualization.
Gordon’s Functional Health Patterns Model
A method used to aid in nursing diagnosis through a structured assessment.
