Delusion
Fixed false belief.
Hallucination
False sensory perception with no real external stimulus.
Illusion
Misinterpretation of a real external stimulus.
Flight of ideas
Rapid shifts between topics.
Loose associations
Illogical connections between thoughts.
Tangential thinking
Never returns to the main point.
Circumstantial thinking
Includes excessive detail but eventually returns to the point.
Blunted affect
Reduced emotional expression.
Flat affect
Total absence of emotional expression.
Labile affect
Rapid, unpredictable mood swings.
Neologism
Word invented by client with meaning only to them.
Perseveration
Stuck on a single idea; cannot move on.
Waxy flexibility
Maintains an awkward physical position for an extended period.
Word salad
Incoherent mixture of unrelated words and phrases.
Thought blocking
Sudden interruption in train of thought.
Thought broadcasting
Belief that others can hear one's thoughts.
Thought insertion
Belief that thoughts are placed into one's mind by an outside source.
Thought withdrawal
Belief that thoughts are being removed by an external force.
Ideas of reference
Belief that random events have special personal meaning.
Depersonalization
Client feels they themselves are unreal.
Derealization
Client feels their environment is unreal.
Akathisia
Motor restlessness, often a medication side effect.
Catatonia
State of immobility, stupor, or extreme excitement.
Mood
What the client states (subjective emotional state).
Affect
What the nurse observes (objective emotional state).

Diagnostic and Statistical Manual of Mental Disorders, Fifth Edition, Text Revision, used for psychiatric diagnoses.
CIWA-Ar
Clinical Institute Withdrawal Assessment for Alcohol; a scale to assess alcohol withdrawal severity.
Wernicke Encephalopathy
Acute thiamine deficiency; requires immediate IV thiamine.
Korsakoff Psychosis
Chronic, permanent sequelae of untreated Wernicke's; involves memory loss and confabulation.
Flumazenil (Romazicon)
Antidote for lorazepam (Ativan); used for benzodiazepine overdose.
Cholinesterase Inhibitor
Class of drugs that increase acetylcholine levels; used in Alzheimer's.
Epinephrine
Antidote for anaphylaxis due to IV thiamine administration.
Neuroleptic Malignant Syndrome (NMS)
High fever, severe rigidity, autonomic instability associated with antipsychotic use.
DBT (Dialectical Behavior Therapy)
Gold standard for treating borderline personality disorder.
Agoraphobia
Fear of open/public places, often linked to panic disorder.
OCD (Obsessive-Compulsive Disorder)
Characterized by obsessions (intrusive thoughts) and compulsions (ritualistic behaviors).
Antisocial Personality Disorder
Lack of remorse or empathy; deceitfulness and manipulation.
Schizophrenia
Chronic mental disorder characterized by hallucinations and delusions.
Bipolar Disorder
Mood disorder characterized by episodes of mania and depression.
Generalized Anxiety Disorder (GAD)
Excessive worry occurring more days than not for at least 6 months.
