Apomorphine
A direct dopamine receptor agonist used to treat erectile dysfunction (ED) by stimulating dopamine receptors in the brain.
Mechanism of Action
Apomorphine activates dopamine D1 and D2 receptors in the hypothalamus and ventral tegmental area to increase penile blood flow and facilitate erections.
Dosage
Recommended starting dose of apomorphine for ED is 2 mg, taken sublingually 25-30 minutes before sexual activity.
Efficacy
Clinical trials show apomorphine is effective in approximately 60-70% of cases, especially in psychogenic ED.
Apomorphine vs. PDE5 Inhibitors
Apomorphine is best for psychogenic ED or when PDE5 inhibitors fail; PDE5 inhibitors are more effective for physically rooted ED.
Side Effects
Common side effects of apomorphine include nausea, dizziness, yawning, and nasal congestion.
Contraindications
Apomorphine should be avoided in patients with severe orthostatic hypotension, cardiovascular disease, or hypersensitivity to dopamine agonists.
Administration
Apomorphine is administered sublingually, allowing for rapid absorption, typically working within 25 minutes.
Effect on Psychogenic ED
Apomorphine is particularly effective for erectile dysfunction caused by psychological factors such as anxiety or stress.
Clinical Trials
A landmark 2001 study found that 68% of men with psychogenic ED achieved sufficient erections after taking apomorphine.
