gluteus maximus origin

origin: cocyx, sacrum edge, posterior illiac crest of pelvis

insertion: iliotibial tract and gluteal tuberosity (lower fibers)

gluteus medius

origin: Gluteal surface of ilium, between posterior and anterior gluteal lines, just below the iliac crest

insertion: lateral aspect of greater trochanter of femur

gluteus minimus

origin: gluteal surface of ilium between anterior and inferior gluteal lines

insertion: anterior aspect of greater trochanter of femur

tensor fascia latae

origin iliac crest, posterior to ASIS

insertion: iliotibial tract that attaches to lateral condyle of tibia

sartorius origin

origin: anterior superior iliac spine

insertion: proximal, medial surface of tibia

adductor magnus

origin: pubic tubercle

insertion: medial lip of linea aspera and adductor tubercle of femur

adductor longus

origin: pubic tubercle

insertion: Pectineal line and medial lip of linea aspera

adductor brevis

origin: inferior ramus of pubis

insertion: pectineal line and medial lip of linea aspera

pectineus

origin: superior ramus of pubis

insertion: pectineal line of femur

gracilis origin

origin: inferior ramus of pubis

insertion: Proximal, medial shaft of the tibia at pes anserinus tendon

semimembranosus

origin: ischial tuberosity

inserion: posterior aspect of medial condyle of tibia

semitendinosus

rectus femoris

origin: anterior inferior iliac spine

insertion: tibial tuberosity

vastus lateralis

origin: lateral lip of linea aspera, gluteal tuberosity, & greater trochanter

insertion: tibial tuberosity

vastus medialis

origin: medial lip of linea aspera

insertion: tibial tuberosity

vastus intermedius

origin: anterior and lateral shaft of femur

insertion: tibial tuberosity

tibialis anterior

origin: lateral condyle of tibia, proximal lateral shaft of tibia & interosseous membrane

insertion: plantar (inferior) surfaces of medial cuneiform & base of 1st metacarpal

extensor hallucis longus

origin: middle anterior surface of fibula & interosseous membrane

insertion: dorsal base of distal phalanx of 1st toe

extensor digitorum longus

origin: lateral condyle of tibia, proximal anterior shaft of fibula & interosseous membrane

insertion: middle and distal phalanges of 2nd-5th toes

gastrocnemius

origin: femoral condyles (posterior surface)

insertion: posterior aspect of calcaneus

soleus

origin: soleal line of tibia, posterior surface of tibia & posterior aspect of head of fibula

insertion: posterior aspect of calcaneus

plantaris

origin: lateral supracondylar line of femur

insertion: posterior aspect of calcaneus

tibialis posterior

origin: proximal posterior tibia shafts of of tibia and fibula & interosseous membrane

insertion: all 5 tarsal bases and 2-4 metatarsal bases

flexor digitorum longus

origin: middle posterior surface of tibia

insertion: plantar (inferior) base of distal phalanges 2-5

flexor hallucis longus

origin: middle half of posterior fibula

insertion: base of distal phalanx of 1st toe

fibularis longus

origin: head of fibula and proximal 2/3 of lateral fbiula

insertion: plantar (inferior) base of 1st metatarsal medial cuneiform

fibularis brevis

origin: distal 2/3 of lateral fibula

insertion: dorsal surface of tuberosity on lateral sid eof 5th metatarsal
