outcome/service delivery model examples

Leisure Ability Model

Health Protection/Health Promotion Model
goal of TR is for clients to achieve the highest level of heath and well being. the purpose of TR is identified as enabling a client to recover from a threat to health and to achieve optimal health. aims to reduce barriers to health (not recreation).
Health Protection/Health Promotion Model: prescriptive activities
provide stabilization and energy. these are short activities that provide the client with a sense of control mostly prescribed by the CTRS
Health Protection/Health Promotion Model: recreation activities
done collectively between the CTRS and the patient. these activities are designed to help get the patient re-engaged and potentially to reduce symptoms and optimize their health. can also teach new life skills
Health Protection/Health Promotion Model: leisure
the end stage. this is where the client has the greatest amount of freedom and choice. the client feels a sense of purpose and motivation which leads to optimal health and well being
TR Service Delivery and TR Outcome Model
go together. the first directs the type of services to be designed and offered. the second is an extension of the first. the end result is to increase quality of life by increasing functional capacity
TR Service Delivery Model

Self Determination and Enjoyment Enhances
based on the thought process that self-determination and enjoyment can lead to functional improvement and self-growth. by focusing on self-determination, enjoyment and functional improvement makes it align well with the health and human services emphasis
after adapting a practice model
