Turn on faucet with power towel

Count off/communicate
Pivot on strong side
Lower pt to w/c with ease
Place feet in footrests
Check for safety, comfort, body alignment
Wash hands Check bed wheels are locked
Place bed in lowest position
Lock w/c, explain procedure
Apply gait belt and non skid footwear
MAKE SURE W/C IS ON CORRECT SIDE OF BED
Move foot rest, assure pt is pivoting on strong side
Pt placed in bed correctly
Removed footwear, gait belt, and covered with blanket
Check for safety, comfort, and body alignment
Make sure to give pt call bell
Clean up, remove equipment and wash hands
Change a pt gown (right sided hemiparesis)
Wash hands
ID patient (check both name and wristband)
Introduce yourself, explain procedure
Retrieve clean gown
Provide privacy (pull curtain and cover pt with bath blanket)
remove it from the strong side then weak side
put it on weak side then strong side
Vital signs
Wash hands and gather equipment
Identify patient
Name, write it down
DOB, write it down
Explain procedure
Radial pulse
Locate it
Palpate for 60 seconds
Verbalize and record result
Respirations
Make sure pt is not aware
Verbalize start and end time of 60 seconds

Blood pressure
Clean stethoscope
Locate brachial pulse
Make sure cuff is placed correctly
Make sure stethoscope is placed correctly

Assist pt to turn around
Lift height bar
Assist off platform PRN

Stand still, do not remove shoes
