Medicare A pays 100% for 20 days, 80% for days 21-100

1-2 times/week x 3 weeks

2 times/week x 60 min x 4 weeks

re-eval required every 30 days

3 times/week x 4-6 weeks for 60-75

Pediatric acute care

Frequency will be determined by IEP and will remain the same until IEP modified
