Health Program
Consists of a variety of planned activities to address the assessed health concerns of clients over time
Builds on client strengths in order to meet specific goals and objectives
Aims to positively impact knowledge, attitudes and behaviours

PRECED-PROCEED MODEL
Program Logic Model
PRECEDE - PROCEED MODEL
A complex but comprehensive planning model that has 9 phases.
Consists of a Preceed and Proced phase.
It emphasizes two basic assumptions:
Health and health risks are caused by multiple factors
Efforts to effect behavioural, environmental, and social change must be multidimensional or multisectoral and participatory in that the target audience is actively involved in the model
Precede Phase
Phase(1-5): Sets directions and objectives for the ensuing phases
Proceed Phase
Pases (6-9): Adressess the need for health promotion interventions and approaches used to change unhealthy behaviours

Outcome Evaluation
Program Logic Model
Used in health care as a planning and evaluation framework
Depicts the components in diagrammatic form
Depicts a cause-and-effect sequence or path toward a stated outcome
Clarifies logical linkages of program inputs, outputs, and outcomes related to a specific health concern or situation
Formative Evaluation
Assessing whether objectives are met or planned activities completed
Begins with an assessment of the need for a program
Occurs on an ongoing basis while program exists
Summative Evaluation
Assessing program outcomes or as a follow up of the results of the program
Looks at the end result of the program
Evaluates program effectiveness and impact
