How to assess neonate based on how they present (what are priority actions based on presentation)
Difference between neonates and adults
Neonates are belly breathers, breathe more with nose, body temperature regulation, has higher hg averages, anatomy of heart is different, they can’t produce blood clots, elevated WBC
When a newborn is born what is done to the baby (where is the baby put)
Incubator, provides warmth, preheated radiant warmer
Course of action your gonna do for newborn after delivery
Count respirations per min, suction mouth and nose with bulb syringe to clear mucus, monitor respiratory effort, observe abdomen because newborn breathing involves use of diaphragm and abdominal muscles
List in order what the external stimuli that indicates neonatal breathing
1) Chest compression during birth draws air into lungs
2) Touch, sound, light, cool air
3) stimulate respiratory center
4Infant takes first breath
What happens to the heart chamber when baby takes its first breath?
Decreased pressure in the right ventricle, increased pressure in left atrium, left ventricle, and aorta
Foramen ovale closes at birth when increased pressure in left atrium and decreased pressure in right atrium occur
Constant circulation leads to permanent closure of foramen ovale within few months
What do you want to consider for direct care after birth of a term baby (expected findings)
First stool is black and sticky, temperature within 97.7F-98.9
thermoregulation
Normal parameters for newborn kidney renal function
Urine will be odorless light in color or clear, within 24hrs of birth should urinate ab 15mL , first 2 days of life daily output should be 30-60mL, volume should be 300 mL per day if taking more breast milk or formula, urine doesn’t concentrate well until baby is 6 weeks old
What does normal newborn stool look like
Black and sticky
Jaundice in newborn
Bilirubin=jaundice=yellow skin
To much RBC that breaks down so bilirubin is released as waste product.
There was lot of RBC cuz of stress of labor
Learn to C.U.S
physiological jaundice doesnt appear until 2-4 days (yellow eyes or skin)
Therapeutic communication for parents that ask about the skin color of the child (specifically if yellow)
You have Simpson baby cuz when have to much red blood cells, it break and releases special thing called bilirubin which is waste product of cell so congrats you have Asian baby
What is indirect bilirubin?
Waste product of RBC that is released, Causes jaundice
What is the baby immunity
Protect newborn from infection
Screenings visitors for illness
Hand hygiene
Encouraging parents to being immunizations at 2 months of age
Doesn’t produce own antibodies until 2 months of age
What does normal neonate behavior look like
Crying, quiet when soothed, engage in mutual gazing, focus eyes and track object or person they are interested in, if they don’t like something they will yawn, cry, or turn away
