ICDAS: Cavitated vs Non Cavitated

ICDAS: Cavitated vs Non Cavitated

  • ICDAS 1-3 initial lesion → if active → sealant/ SDF = noninvasive or resin infiltration = microinvasive

  • ICDAS 3-4 microcavitation → usually resin infiltration bec microcavitated

  • ICDAS 5-6 Cavitated → SDF or restorative treatment

resin infiltration vs sealants

  • resin infiltration prevents acid demineralizing tooth gets absorbed

  • sealant is different bec just cover tooths

Resin Infiltration (D 2990)

  • minimally invasive dental treatment for white-spot lesions and early-stage enamel decay that uses a low-viscosity resin to fill microscopic pores in the tooth.

  • This process arrests the progression of decay and improves the tooth's appearance without drilling

5
New cards

enamel demineralization

  • Sub-surface lesion under pseudo-intact

    surface layer

    Increased pore volume (up 30%)

    Pores are diffusion path for

    microorganisms and substrate

    Light scattering causes white spot

6
New cards

resin infiltration of demineralized enamel :

1) Removal of surface layer with acid: etched the pseudo intact surface layer 2 min 15% HCl

2) Pores are filled with resin

3) Occlusion of diffusion paths: resin infiltration has LOW runny viscosity

4) Inhibition of lesion progression

5) Stabilization of lesion: can increase surface hardness with resin

indications for interproxmial resin infiltration

E1, E2, D1

NOT D2 and D3 or inner middle third dentin

resin infiltration of interproximal lesions steps:

1) rubber dam and wedge

2) 15% HCL ACID ETCHING (120 S)

3) rinse and EXCESSIVE drying 30 sec bec enamel only so no collagen fibers will collapse

4) use 100% ethanol drying agent to take out water from lesion

5) apply resin (TEDGMA) after excessive drying for 3-5 min

6) light cure each side 40 sec

7) add 2nd resin surface to fill in irregularities

8) can add glycerin gel to get rid of oxygen inhibited layer

New cards

Resin Infiltration of white spots:

  • how white spot is masked: must have enamel and resin with similar Refractive Index so light will go thru and not reflect

  • Apatite RI: 1.62

    Mircro-Porosities

    Air RI: 1.0

    Water RI: 1.33

    Infiltrant RI: 1.46
