P2 Fall Exam 1 - Dosages

Baclofen

Initial: 5mg po tid; increase by 5mg per dose every three days Max daily dose: 80mg/d

Carisoprodol

250-350 mg po tid

Codeine

15-60 mg po q4h. Do not exceed 4000 mg of Tylenol

Cyclobenzaprine

Initial: 5mg po tid, may titrate up to 10 mg po tid for 2-3 weeks (IR)
Initial 15 mg po daily may titrate up to 30 mg a day

Fentanyl

One patch every three days (up to 100mcg/h), may remove after two days and replace with new patch

Hydrocodone

10mg bid, may titrate to response

Methadone

2.5 mg po q8h-q12h may titrate to the response. (usual range 80-120 mg/d)

Methocarbamol

1500 mg po qid qid for 2-3 days. Max of 6000mg.

Celecoxib

100-200 mg po bid or 400 mg qd

Diclofenac

IR - 18-50mg po tid. ER Max of 200 mg/d

Ibuprofen

OTC max 1200 mg/d. Rx: 3200 mg/d in three or four doses.

Meloxicam

15mg/d

Morphine

10-20 mg bid

Naproxen

Max 1000 mg/d (ER), 250-550 mg bid is max for IR

Oxycodone

5-15 mg po q4-6h. ER is 10 mg po q12h

Tizanidine

2 mg po tid up to 8mg qid Max is 32 mg/d

Sumatriptan

Max: 200mg/d (up to 100mg a dose every 2 hours as needed for migraine). 40mg/d nasal. SubQ is 12mg/d. Transdermal is 2 patchs/d.

Tramadol

50mg po q4-6h. May go up to 400 mg/d. ER: 100mg po daily. May go up to 300 mg/d

buprenorphine/naloxone

12-16 mg qd sublingual. Titrate up to 4-24 mg/d
