Long-term care (LTC)
Care given in facilities for people who need 24-hour skilled care.
Skilled care
Medically necessary care provided by a skilled nurse or therapist, available 24 hours a day.
Nursing homes
Facilities once widely used to refer to long-term care settings, now commonly referred to as LTCFs or SNFs.
Chronic conditions
Medical conditions that last a long period of time, often throughout a person's life.
Residents
People who live in long-term care facilities, as the facility is their home during their stay.
Terminal illness
An illness that will eventually cause death.
Length of stay
The number of days a person stays in a healthcare facility, which may vary from a few days to longer than six months.
Rehabilitation centers
Facilities that provide therapy and care to individuals recovering from illness or injury.
Diagnoses
Medical conditions determined by a doctor that affect the level and type of care a person requires.
